Safe work procedures that include lockout procedures are required. Accumulation of waste material (metal chips) on the floor is not safe and contrary to the safety manual suggestions. See the inspection report from June 21, 2024 for details of the original incident.

A worker was cleaning debris on a horizontal metal band saw and injured their finger. The worker was transported by ambulance to the hospital for further medical assessment

During the disposal process three workers were exposed to the contents in the bags, which was later determined to be a product containing a hazardous chemical. All workers were transported to hospital for follow up.
